Imperforate Hymen

Imperforate Hymen is one of the gynecological problems found in young women. In case of imperforated hymen, the opening of hymen is hardly present. Depending on the development of external and internal genitalia of female foetus, the hymen is either absent or obstructs the vaginal opening. Women with imperforate hymen often suffer from severe pain during menstruation with no bleeding.

In most cases the diagnosis of imperforate hymen occurs during neonatal examination. Such problem is also evident in the teenagers who suffer from cyclic and acyclic pelvic pain, primary amenorrhea, hematocolops and bulging hymen. Extensive researches made on such gynecological problems have proved that imperforate hymen are often inherrent and occurs as a result of failure of central epithelial cells of the membrane of hymen. Sometimes these problems may occur due to inflammatory causes.

Sometimes imperforate hymen occur in the teenagers if the uterus and vagina are not dilated by estrogen. The menstrual material conglomerate near the vaginal tract resulting in severe abdominal and pelvic pain. In some women “os uteri” merge together and continue to be in this state until the beginning of menstrual cycle. With menstruation the vaginal tract are forcibly opened.

Imperforate hymen are more common in the young girls, specially those below 15. The expansion of vagina and shift of uterus may result in the extension of cystic mass in the abdominal tract. Such problem is often associated with pain in lower abdomen and other urinary problems. If the diagnosis is not made at the initial stages then the patient is subjected to radiological evaluation. If the external genitalia is properly examined, then the presence of a mass between labia majora will be evident. The mass which has a bluish red appearance may differ in size. Such mass may often create a pressure leading to constipation.

Treatment of Imperforate Hymen

Women suffering from such problem should contact a gynecologist as early as possible. Only a gynecologist or specialist plastic surgeon will be able to cure the problem after perforating the hymen surgically.

Surgical therapy which includes hymenotomy is recommended for treatment of Imperforate Hymen. Needletrip electrocautery helps in hemostasis of hymenal edge. Yankauer suction tip is also used for treatment of imperforate hymen. They are usually inserted through the vagina or through the cervix for proper excretion. Evacuator of high speed is sometimes required for proper treatment. Care must be taken to prevent infection in the post operational phase.
